I think it's after an update; the controller went offline and couldn't get it online. I tried multiple resets and restarts; none of them did the trick. Just thinking about whether anyone else has had the same issue as me and how you solved it.
Ever had that, when it updated from 1.6.006 to 2.0.003, at that time, the reset and restart did the trick. They told me something like putting the phone close to the device while adding, unplugging the controller couple seconds then plug it backin. It succeeded finally anyway.
